What to Expect

1

Complete Your Online Evaluation

Start with a brief, private screening and a secure medical intake — no card required to begin.

2

Provider Review

A licensed nurse practitioner reviews your complete health history to determine whether treatment is appropriate.

3

Personalized Treatment Plan

If appropriate, you receive a plan built around your goals, health, and history — not a one-size-fits-all prescription.

4

Ongoing Clinical Support

Your provider monitors your progress and adjusts your plan over time, with direct access when you need it.

Medication options

Purelivity’s program uses GLP-1 based therapies. Semaglutide or tirzepatide may be prescribed when clinically appropriate. Eligibility and medication selection are determined by the treating provider after a complete medical evaluation. Medication is only one part of a comprehensive weight-management plan.

Laboratory testing

Routine baseline laboratory testing is not automatically required for every patient. Your provider may request laboratory testing or additional medical records when clinically indicated based on your health history, medications, symptoms, or risk factors.

Safety & eligibility

Purelivity’s GLP-1 program is intended for adults with a BMI of 30 or greater, or a BMI of at least 27 with a qualifying weight-related health condition. Meeting these preliminary criteria does not guarantee treatment or a prescription. Eligibility is determined through a complete medical evaluation. Purelivity applies clinic-specific safety standards that may be more restrictive than medication labeling. Additional records or laboratory testing may be requested when clinically appropriate.

Care That Goes Beyond a Prescription

Medication is only one part of lasting change. Purelivity pairs evidence-based medicine with personalized care and practical support, so progress is something you can sustain.

  • Evidence-based medicine
  • Personalized care
  • Lifestyle coaching
  • Long-term success
